Role details

The Home and Community Care at the Mackay Community Health Centre is seeking applications for a Home Care Worker to join their team on a casual basis.

This Expression of Interest collects a variety of information that will help identify potentially suitable applicants for future opportunities.

Your opportunity

To provide high quality support services to frail, older people in order to maximise their independence at home and in the community. To be supported by a highly motivated, consumer focused Home Care Coordination Team, liaising closely with Community Allied Health Professionals.

About the Team

Mackay District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible and goal orientated basic support services. Our service is also an Approved Aged Care Provider, providing Client Directed Home Care packages to eligible consumers.

Mackay District Home Care Service provides basic support services, predominately Domestic Assistance and Social Support to approximately 1000 consumers within the Mackay district.

Potential CHSP consumers are registered and screened by “My Aged Care” and if eligible referred for assessment by a Regional Assessment Service (RAS). Assessment is based on assisting frail aged people, people with a disability and their carers to continue to live in the community as independently as possible.

The Mackay District Home Care office is located in the Carlyle Community Health Centre, 40 Carlyle Street Mackay.
