The Senior Case Manager Sentence Management Services (SMS) is responsible for assessment of offending risk and need, development and enactment of an engagement plan in accordance with the individual’s assessed risk and need to improve rehabilitation, reintegration and/or community supervision outcomes for incarcerated individuals.

The Senior Case Manager SMS will use a whole-of-sentence planning approach applying the principles of Risk Need Responsivity (RNR) to case management and will be responsible for coordinating activities to support individuals’ readiness for release and transition back to the community.

In this role you will:

  • Administer evidence-based validated assessment tools to identify risk and need and facilitate system pathways to promote community safety;
  • Develop, implement and update an individualised Engagement Plan in collaboration with an incarcerated individual, their families/support network and internal and external service providers;
  • Promote and facilitate incremental and positive behavioural and attitudinal change using effective case management strategies and drawing on strengths-based approaches including motivational interviewing, brief intervention and referral to appropriate services and programs in accordance with the Engagement Plan;
  • Undertake direct interaction and manage a caseload of incarcerated persons, including high risk sexual, violent and other priority groups;
  • Undertake complex assessments of risk to the community, including identification of outstanding treatment and reintegration needs, evaluation of relapse prevention plans and consolidation of learning from offending behaviour interventions, and preparation of high quality reports to inform parole release decision-making by Parole Board Queensland and Commonwealth Attorney-General's Department;
  • Deliver parole related correspondence to incarcerated persons on behalf of Parole Board Queensland and Commonwealth Attorney-General's Department;
  • Maintain a high level of knowledge and understanding of contemporary offender assessment and management approaches and practices for specific offending portfolios, including sexual and violent offending, to improve community safety;
  • Participate in practice supervision and professional development activities consistent with contemporary evidence-based practice and agency policies and procedures;
  • Provide high level and timely advice in relation to risk and need, case management, rehabilitation and release planning and parole suitability for incarcerated individuals;
  • Develop and maintain effective relationships with key stakeholders and participate in multidisciplinary case conferences to assist individuals to progress through the correctional system;
  • Ensure continuity of service provision across custody and the community by liaising with community corrections to facilitate the seamless transition of individuals to and from custody;
  • Identify and respond appropriately to risk including liaising with relevant managers, staff and external stakeholders;
  • Support the development of peers and delivery of quality assessment and case management through peer verification, mentoring and coaching of staff; and
  • Show a genuine commitment to invest in the community and understand remote protocols including the multicultural offender profile and promote a positive image of QCS.
