About your role:
- Using a biopsychosocial model conduct mental health assessments and assist consumers and families/ supports to develop a comprehensive individual recovery plan and facilitate the effective delivery, monitoring and evaluation of this plan.
- Work with families and communities to promote recovery and community inclusion and to access generic supports, opportunities, and resources.
- Develop and maintain collaborative work practices and relationships with other agencies (including non-government organisations), service providers and consumer and carer groups to ensure consumers have access to an integrated service system.
- Provide input into the development of evidence-based clinical policy and practice, service development, quality improvement activities, research and evaluation projects and the utilisation of mental health information systems related to service delivery, performance, and outcome measurements.
