About you:

In your new role you will:

• Accountable for service performance against key performance indicators as outlined in CHHHS's service agreement with the Department of Health, including the development of relevant data sets and specific performance indicators for the services and functions of Community Health services against KPIs as agreed with the Executive Director Allied Health.
• Develop and foster strong relationships with the community and community groups within CHHHS's catchment to understand their needs and issues relating to health care delivery.
• Lead the development of operational plans for Community Health Services through strong governance, risk management and accountability, and provide to the relevant parties timely and accurate reporting with a focus on performance outcomes.
