Our goal is to identify and assist the frail older person, who is at risk of early or premature admission into an acute or long-term health facility, to remain living safely and independently in their own home for longer.

About you:

In your new role you will: 

• Work within your professional scope of practice, competencies and ethical standards, following evidence-based practice.
• Demonstrated ability to work autonomously with minimal supervision requirements and preferred experience in equipment prescription, minor and major home modifications.
• Deliver high level Occupational Therapy clinical services within the community setting including clinical problem solving and effective coordination of a complex clients including assessment and intervention, that is culturally appropriate, and client focussed.
• Provide leadership, precept staff and students and contribute to professional practice.
