The Transition Care Program are seeking an Occupational Therapist to join them. In this role you will deliver effective and efficient provision of occupational therapy services to older people, and their carers, partners and families, who have been discharged from acute and sub-acute care settings and accepted into the Transition Care Program.

About you:
In your new role you will;
- Provide consistent, safe and efficient occupational therapy and case management services, within a multidisciplinary team, with infrequent need for direct clinical practice supervision.
- Demonstrate resilience and problem-solving abilities while working in varied and demanding caseload.
- Provision of general clinical advice to professional and operational supervisors, relevant service managers and other stakeholders including non-government organisations and remote community agencies within Far North Queensland, regarding service delivery and case management on a regular basis and with infrequent need for supervision.
