What's involved – The Palliative Care Coordinator Clinical Nurse Consultant (PCC CNC) is an experienced nurse with expert clinical knowledge and an in-depth understanding of Palliative Care, symptom management, and end of life care planning. The PCC CNC employs advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience for individuals and their support persons.
As a member of the Primary Health Team, the role will work collaboratively and more broadly with the team and across Central West Hospital and Health Service (CWHHS) to ensure continued focus on strategic priorities and delivery of operational imperatives.
