About the Child and Youth Community Health Services 

The Child and Youth Community Health Services (CYCHS) brings together a variety of specialist community services to help our kids and their families to lead healthier lives.

CYCHS predominantly provides front line health care to communities throughout south east Queensland. In fact, the service provides access to community care to around 470,000 children, which represents 42 per cent of Queensland's children. A range of services are provided across the continuum of care as well as health promotion.

A number of Child and Youth Community Health programs, including Deadly Ears, reach communities throughout the entire state.

The service's 450 (full time equivalent) staff work in and from more than 50 community health centres, plus clinics, hospitals, schools, shops and offices, covering a geographic area beyond Beaudesert in the south to Kilcoy in the north.

Your Opportunity:

The role of Nurse Navigator – Navigate Your Health will be to provide advanced clinical and professional practice, facilitating a safe and efficient patient journey across the continuum of care to support children and young people from both Child Safety and Youth Justice pathways.  This will be facilitated through applying the Nurse Navigator principles of coordinating patient centred care, creating partnerships, improving patient outcomes and facilitating systems improvement. The role will improve complex patient care, increase system efficiencies, and reduce costs by streamlining and coordinating care. 

Noting both the high levels of unmet health needs and over-representation of Aboriginal and Torres Strait Islander, and Maori/Pacific Islander children and young people in the child safety and youth justice populations, the Nurse Navigator – Navigate Your Health (identified) will also play a key role in ensuring these populations' healthcare needs are supported by the Navigate Your Health service in a culturally appropriate way that builds strong trust and engagement with the patient, family, carers and broader community network.

The Nurse Navigator positions will be appointed within Children's Health Queensland's Division of Child and Youth Community Health Services and located across multiple locations including (but not limited to) South Brisbane, Logan, and Cairns).  The role will have face-to-face engagement with in-scope Child Safety Service Centres, Youth Justice Service Centres, General Practice and other Primary Care Services including Aboriginal and Torres Strait Islander community health services. There will be some local travel required to visit these locations.

In alignment with the Office of the Chief Nursing and Midwifery Officer's Nurse Navigator Role Principles, the position of Nurse Navigator (Navigate Your Health) will work across multiple specialities, support nurse led service models and augment existing models of care across services, facilities and/or clinical areas as outlined in the roles and responsibilities section of this role description.

For this job it is a genuine occupational requirement that it be filled by an Aboriginal and/or Torres Strait Islander person as permitted and arguable under Sections 25 and 104 of the Queensland Anti-Discrimination Act 1991.
