About the Team
Navigate Your Health is a diverse team of Nurse Navigators that delivers person-centred, trauma informed care to children and young people who are in contact with Child Safety and/or Youth Justice systems. Navigate Your Health works in partnerships with families, and their care teams to improve health outcomes, create positive, culturally safe experiences that build trust and empower children, young people, families, and their care teams. The Nurse Navigator will facilitate the health care journey for children and young people in contact with Child Safety and/or Youth Justice systems
About the Role
As a Navigate Your Health Nurse Navigator you will bring an in-depth knowledge of the unique health needs of children and young people in contact with Child Safety and/or Youth Justice systems.
The role of Nurse Navigator (Navigate Your Health) is to provide advanced clinical and professional practice, facilitate the safe and efficient patient journey across the care continuum by applying the Nurse Navigator key principals of coordinating patient centred care, creating partnerships, improving patient outcomes, and facilitating systems improvement.
The Nurse Navigator will provide face-to-face engagement and collaboration with Child Safety, Youth Justice, General Practice, and a variety of other Primary and Tertiary Health Care Services including Aboriginal and Torres Strait Islander community health services and Non-Government Organisations.
In this role you will coach and support others to deliver for children, young people and families (Leader of Others) in accordance with the Children's Health Queensland Leadership Excellence Framework and the Queensland Public Service and the CHQ values.
