About your opportunity
The Nurse Practitioner (NP) Nurse Navigator Service, Community and Primary Health Care provides expert nursing care within a specialty area within the Mount Isa community and the North West Hospital and Health Service, working autonomously and in collaboration with the interdisciplinary health team. The Nurse Practitioner (NP) is responsible for providing advanced professional and clinical leadership in the coordination and delivery of comprehensive and complex care, education and consultancy and portfolio management to ensure safe practice and high quality outcomes.

Supporting a patient's entire healthcare journey

The Nurse Navigator role employs advanced clinical and professional practice capabilities to deliver a positive patient centred health experience for individuals. The nurse navigator function is a fundamental part of the nursing role. This role aims to facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating, empowering individuals and coordinating care.

It is a requirement that the Nurse Navigator has the capability of working with complex caseloads including those with multiple co-morbidities and high frequency presenters across the spectrum of all speciality services and ages. Coordinating patient care within the acute sector; community health and the home setting is essential to the role.

Generous remuneration package comprising salary range of $127,524 to $133,136 p.a. (Nurse Grade 8), 17.5% leave loading, 12.75 % employer contribution to superannuation.
