About your opportunity

This position forms part of a state-wide Nurse Navigator model of care designed to provide holistic support to patients with complex health conditions requiring advanced level of healthcare coordination and works across system boundaries to actively improve health literacy in close partnership with patients and families and other health and allied health services to improve the patient's healthcare experience and quality of life.

The role aims to facilitate a patient's journey through an increasingly complex health system by: reducing fragmentation, mitigating barriers, educating and empowering others, and coordinating care.

What we are seeking:

  • Demonstrated specialised clinical expertise for Nurse Navigator Service at an advanced nursing practice level, by participating directly in the delivery, coordination and management of person centred nursing care producing positive patient experience and outcomes.
  • Taking accountability and responsibility for ensuring practice is evidence based; participating in research and continually monitoring and evaluating nursing activity in the specialty area in relation to the service's performance indicators, clinical governance and relevant standards.
  • Supporting the Nurse Navigator Service to be adequately and appropriately resourced regarding material, financial and human resources though participation in the development and application of the Business Planning Framework methodology.

Requirements:

  • There is an expectation that applicants will be working towards, or interested in, an appropriate post graduate Master of Nursing - Nurse Practitioner course.
  • Proof of qualification, registration with AHPRA and a current practising certificate are mandatory requirements.
  • The position may involve travel to various facilities or units within the North West Hospital and Health Service.
  • Class C motor vehicle licence.
  • This is a Vaccine Preventable Diseases (VPD) risk role.

What's on offer?

Permanent full-time role, 76 hours per fortnight.

Generous remuneration package value up to $144,228 p.a. comprising:

  • salary range between $116,092 to $126,037 p.a. (Nurse Grade 7)
  • annual recreational leave p.a. with 17.5% loading
  • 12.75% employer contribution to superannuation.
