The role

Mount Isa Community and Primary Health Care provide expert support, advice and collaborative planning to ten facilities that comprise the North West Hospital and Health Service region. It promotes the Enhanced Model of Primary Health Care as the preferred framework for delivery of rural health services, through training, education, strategic planning and resource allocation.

The Nurse Navigator Service is a state-wide model of care designed to provide support to patients with complex health conditions requiring a high degree of clinical care. This integrated and proactive approach works across system boundaries, including close partnership with patients and families, other health and allied-health services ensuring patients receive timely and appropriate care.

This position forms part of this state-wide model of care and is responsible for providing advanced professional and clinical leadership in the coordination and delivery of comprehensive and complex care, education and consultancy and portfolio management to ensure safe practice and high quality outcomes.

The role will contribute to positive patient outcomes by practising at an advanced level demonstrated by applying expert knowledge, advanced communication and complex decision-making skills and clinical competencies for extended practice relevant to clinical context of chronic cardiovascular disease encompassing heart failure, coronary artery disease, arrythmia management, cardiac risk factor management, structural heart disease in adult cardiology and rheumatic heart disease.

Mandatory requirements are a Master of Nursing - Nurse Practitioner qualification and appropriate registration/licences. Pre-employment screening, including criminal history checks and Vaccine Preventable Diseases (VPD) risk role compliance, will be conducted for the successful candidate.
