The role

Community and Primary Health Care (C&PHC) services provide expert support, advice and collaborative planning to ten facilities that comprise the North West Hospital and Health Service region. It promotes the Enhanced Model of Primary Health Care as the preferred framework for delivery of rural health services, through training, education, strategic planning and resource allocation.

The Nurse Navigator Service is a state-wide model of care designed to provide support to patients with complex health conditions requiring a high degree of clinical care. This integrated and proactive approach works across system boundaries, including close partnership with patients and families, other health and allied-health services ensuring patients receive timely and appropriate care.

The Mount Isa Sexual Health (MISH) service is a clinical and secondary prevention service sitting within C&PHC and provides sexual, reproductive, men and women's health services including HIV and viral hepatitis management. There is a key focus on the health of young people. Services are provided within Mount Isa and in the outer communities as well as providing support and frameworks or systems to these communities to enhance self-management where applicable.

This role will contribute to positive patient outcomes by practising at an advanced level demonstrated by applying expert knowledge, advanced communication and complex decision-making skills and clinical competencies for extended practice relevant to the NP scope of practice clinical context and specialty.

Mandatory requirements are a Master of Nursing - Nurse Practitioner degree qualification and appropriate registration/licences. Pre-employment screening, including criminal history checks and Vaccine Preventable Diseases (VPD) risk role compliance, will be conducted for the successful candidate.
