About the role
The North West HHS Rheumatic Heart Disease team works in partnership with the Nurse Navigator Service as part of the Community and Primary Health Care department and other key stakeholders including local medical service providers to ensure clients are reviewed regularily and treatment and appointments are in line with the client care plan.
This role will deliver expert and professional clinical leadership in the provision and coordination of person-centered care as part of a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring advanced level of healthcare coordination.
The role aims to facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating, empowering individuals and coordinating care.
Requirements
NG8: Masters of Nursing and Nurse Practitioner endorsement (AHPRA). 3-5 years' experience in cardiology and management experience is desirable.
NP Candidates: Applicants must be enrolled in a Master of Nursing (Nurse Practitioner) post-graduate qualification that will lead to NP endorsement with AHPRA.
CNC / Nurse Navigator: current AHPRA registration as Registered Nurse.
- Applicants must have experience in the specialty of cardiology.
- Travel is a requirement of the role. C Class Motor Vehicle Licence.
- Pre-employment screening will be conducted for the successful candidate.
- This is a VPD risk role: Hepatitis A/B, Measles, Mumps, Rubella, Varicella, Pertussis, Tuberculosis and COVID-19.
- A 6-month period of probation may apply.

Base salary $144,913 to $151,292 p.a. (Nurse Practitioner - Nurse Grade 8).
Base salary $128,701 to $139,732 p.a. (NP Candidate or CNC / Nurse Navigator - Nurse Grade 7).
In your downtime, we will provide 2 return airfares p.a. (inc. spouse/dependents) to the nearest east coast provincial city in conjunction with your recreation leave; of which we offer 5 weeks p.a. with a 17.5% loading/bonus.
