An exciting opportunity exists for an Aboriginal and Torres Strait Islander Hospital Liaison Officer within the Hospital Liaison Program which supports and works collaboratively with multidisciplinary teams and primary care providers within the Sunshine Coast region to ensure culturally appropriate and safe health services contribute to the improved health and well-being of Aboriginal and Torres Strait Islander patients.
Sunshine Coast Hospital and Health Service (SCHHS) is in southeast Queensland and extends through the coastal and hinterland areas from Caloundra in the south and Gympie in the north.

The Aboriginal and Torres Strait Islander Health Directorate works across all facilities and provides high quality care and support to Aboriginal and Torres Strait Islander patients and their families as well as cultural education and training for SCHHS staff. The Aboriginal and Torres Strait Islander Health Directorate provides a multidisciplinary team approach to complex care including inpatient support, case management and care coordination through the health service and a supported transition back into primary care.
Key Responsibilities include:
- Facilitate and maintain close liaison links with staff to enhance access to health services and practical social support services for Aboriginal and Torres Strait Islander patients and their families.
- Develop linkages with all relevant hospital wards, departments, and community services to facilitate admission, discharge planning and referral to follow-up services.
- Network and liaise between Aboriginal and Torres Strait Islander community organisations, public sector agencies, patients, their families and Aboriginal and Torres Strait Islander Programs within the Hospital and Health Service.
Remuneration value up to $117,817 p.a., comprising salary between $75,090- $103,261 p.a., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HWF3). Applications will remain current for 12 months. Please note: only applications from candidates will be accepted (applications that may result in an agency fee will not be considered).
