The Nurse Navigator facilitates a person-centred care coordination model for patients with chronic conditions and/or complex care needs who are regular health service consumers. This facilitation is achieved by reducing fragmentation, mitigating barriers, educating, empowering and coordinating patient care.

Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be indigenous to the Aboriginal or Torres Strait Islander Community.

This role focuses on supporting our Aboriginal and Torres Strait Islander people with chronic conditions and complex care needs particularly those transitioning between The Townsville Hospital and their local health service.  The opportunity is to assist and reform wide service delivery to improve health care outcomes for this patient cohort with a proactive approach to streamlining their healthcare journey and engaging primary health care services.

Generous remuneration package value up to $140,711 p.a. comprising salary range of $113,261 to $122,964 p.a. (Nurse Grade 7), 17.5% leave loading, 12.5 % employer contribution to superannuation. The ideal applicant will demonstrate the following:

  • Advanced clinical evidence-based skill and knowledge in the care coordination of multiple complex patients and interdisciplinary care management evidenced by recency of practice and achieved clinical and professional competencies.
  • Leadership proficiency through the articulation of team engagement principles and personal attributes that influence a positive team culture and achieve active collaboration across multidiscipline services.
  • Demonstrate proficiency in the establishment of therapeutic relationships, incorporating health education, health literacy, advocacy and culturally competent professional practices that facilitate achieving patient identified goals and optimised health experiences for Aboriginal and Torres Strait Islander people.
  • Applied knowledge of quality improvement frameworks and associated quality processes inclusive of the integration of nursing research, identifying experiences that evidence a professional commitment to contributing to quality improvement.
