The Nurse Practitioner (Diabetes Services) facilitates a person-centred care coordination model for patients with Diabetes, chronic conditions and/or complex care needs who are regular health service consumers. This facilitation is achieved by reducing fragmentation, mitigating barriers, educating, empowering and coordinating patient care.  The Nurse Practitioner will demonstrate clinical expertise, utilise case management principles and primary health care service alternatives, where clinically appropriate, with the aim of reducing avoidable hospital admissions and improving patient outcomes through early and proactive interventions.

As the Nurse Practitioner, your role will involve the following:

  • direct referral of clients to other health care professionals;
  • review of the integrated electronic Medical Record and Insulin Dashboard of inpatients;
  • education of health care professionals, patients and their families;
  • prescribing medications; and
  • ordering diagnostic investigations.

Generous remuneration package value up to $128,295 p.a. comprising salary range of $107,388 to $112,114 p.a. (Nurse Grade 8), 17.5% leave loading, 12.5 % employer contribution to superannuation.
