You are invited to join a dynamic team with Hospital in the Home (HITH) as a Clinical Nurse Consultant. In this role you will deliver an advanced level of professional leadership and clinical expertise relevant to the HITH Service. The CNC must work in collaboration with the multi- disciplinary team to support the clinical unit in the provision of safe quality healthcare external to the hospital environment.

About you:

In your new role you will:

• Provide clinical governance to all HITH patients, including their safe and timely transfer to and from other providers.  
• Monitor and investigate all clinical incidents and actively support risk management processes.
• Develop clinical pathways, models of care and service delivery modes that enhance patients' safety, expand the scope of clinical care delivered, maximise service utilisation and enhance patient flow for the CHHHS.
• Provide expert advice, counsel and leadership on clinical service delivery and development to appropriate committees and forums. 
• Communicate and engage with consumers to promote and improve health literacy related to HITH.
