- You will assess, identify, monitor and review treatment, care and support needs of participants with complex injuries and disabilities.

- You will assist participants to set and achieve individual goals.

- You will support participants to maximise their independence, their participation in the community and ability to engage in meaningful activities.

- You will support participants to participate in decision making and exercise choice and control.

- You will also lead projects and/or participate on projects to provide your expertise in support planning principles.

This role includes managing participants who have an ongoing CTP claim that will require engagement with external stakeholders, claims advisors and insurers.

Your role

  • Provide participants, their family and carers with information and support about the Scheme and the Agency.
  • Assess participant needs and create and amend support plans including making decisions autonomously (or recommendations to the Manager) regarding the necessary and reasonable treatment, care and support of participants with higher complexity needs.
  • Lead and support other team members in decision-making by providing expert technical guidance, coaching, and peer supervision.
  • Undertake peer reviews and to ensure high quality treatment, care and support services for participants.
  • Understand and discharge delegated functions in accordance with the Act.
  • Identify ways to empower participants in the development, implementation and monitoring of support plans, to reflect participant goals, choice and control, and measurable outcomes.
  • Provide options for treatment, care and support, balancing the needs of participants with Scheme requirements, sustainability, and community expectations.
  • Lead and facilitate opportunities for education and capacity building of participants to empower increasing levels of independence towards self management.
  • Manage service requests, approvals and payments in accordance with agreed needs and assessments and plans.
  • Liaise with legal representatives of participants and their families/carers as needed.
  • Liaise and collaborate with stakeholders to ensure successful implementation and maintenance of support plans.
  • Lead or contribute to specific projects or initiatives, policies, processes and procedures to continuously improve services and enhance the quality of outcomes for participants.

About you

To be successful in this role you will demonstrate a strong commitment to shared values, and a broad skillset that includes:

  • High levels of applied knowledge and understanding of support planning principles and processes, for a range of injuries and/or disabilities, including the ability to lead or contribute expert knowledge for projects and continuous improvement.
  • Proven track record to autonomously manage support planning and goal-setting processes of high complexity, whilst discharging delegated statutory functions in accordance with the Act.
  • High level knowledge of insurance management principles and high-level skills in personal injury claim management for catastrophically injured people.
  • Highly developed organisational skills and proven leadership skills to guide and coach within a dynamic environment, displaying a collaborative, autonomous, flexible, and agile working style and approach.
  • Highly developed communication skills and proven ability to build rapport, communicate complex information, negotiate with a variety of internal and external stakeholders, and provide technical and/or people leadership within the team.
  • Proven track record in planning and making decisions and recommendations for complex scenarios based on relevant evidence in relation to disability services.
  • Effectiveness in identifying and responding quickly, flexibly and appropriately to higher complexity issues of participants, their family and carers.
