Role details
The Integrated Health at the Mackay Base Hospital is seeking applications for a Clinical Nurse (General Practice Liaison) to join their team on a temporary basis.

Your opportunity
The Clinical Nurse (CN) provides nursing services within the health service settings; and is responsible for a portfolio with a focus on clinical care/leadership, management, education and/or research. The activities are predominantly clinical in nature but also provide support to a Nurse Grade 7 or above in management activities. The CN provides advanced level clinical skills to ensure patient-focused evidence-based nursing care, plans and coordinates the clinical management of patient/client care and provides leadership and support to other nursing staff.
Reporting to the GPLO, the CN will be responsible for supporting service model initiatives conducted by General Practice Liaison Unit in collaboration with the primary care sector and the Mackay Hospital and Health Service.
Focus will involve assisting with delivery of initiatives including Health Pathways, Clinical Prioritisation Criteria and General Practice Liaison activities and will work closely with external and internal stakeholders.
The incumbent will also support General Practice and Residential Aged Care Facilities to adopt digital service and health information delivery systems to optimise outcomes for patients.
About the Team
The General Practice Liaison Unit (GPLU) sits within the Medical Services at the Mackay Base Hospital. The GPLU works across the Mackay Hospital and Health Service to improve the integration between the hospital and primary health care setting, ensuring care is integrated across the entire patient journey.
