Advanced Health Worker

Contribute to improving health outcomes for First Nations Peoples within the growing West Moreton Region! The purpose of this role is to deliver high level cultural advice and professional and culturally specific support to First Nations adults in West Moreton through the implementation of a range of chronic disease management activities, and through the application of the principles of best practice, education and leadership to ensure safe practice and high-quality client outcomes.
Under s25 of the Anti-Discrimination Act 1991 (Qld) it is a genuine occupational requirement for the incumbent to be Indigenous to the Aboriginal or Torres Strait Islander Community.

What we're looking for
Our ideal candidate will be someone who can demonstrate the following:
- High level interpersonal, communication, consultation and negotiation skills and ability to develop effective partnerships within the Health Service and with external stakeholders.
- Cultural lived experience and understanding of the cultural factors and histories that affect the health and wellbeing of Aboriginal and Torres Strait Islander people today.
- Ability to provide effective cultural support to First Nations peoples accessing health services.
- Ability to support multidisciplinary teams to improve their understanding of the Social, Emotional and Well-Being needs of First Nations consumers.
- Appropriate clinical skills for the delivery of evidence-based health assessments, health promotion, education and illness prevention activities and primary health care services to First Nations peoples.
Mandatory Requirements:
- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
