About the role
Provide advanced clinical nursing expertise within the Intensive Care Outreach Service across Caboolture, Kilcoy and Woodford facilities. This role focuses on delivering specialist care, clinical leadership, and coordination to support early recognition and management of patient deterioration, ensuring safe and high-quality outcomes across inpatient settings.
As a Clinical Nurse Consultant, you will work at an advanced practice level, providing expert assessment, advice, and intervention for patients at risk of clinical deterioration. A key component of the role involves coordinating care between general wards and Intensive Care Services, developing and implementing proactive management plans to reduce preventable deterioration and escalation events.
You will operate with a high degree of autonomy, applying advanced clinical judgement and decision-making while collaborating closely with multidisciplinary teams across the service. The role requires strong leadership capability, acting as a clinical resource, mentor, and advisor to nursing and medical staff, supporting best practice and strengthening workforce capability within deterioration management and critical care.
The position also contributes to broader service improvement and clinical governance activities, including identifying practice gaps, supporting implementation of evidence-based improvements, and participating in education, research, and innovation initiatives. The Intensive Care Outreach Service plays a central role in promoting patient safety, enhancing clinical outcomes, and supporting timely escalation and intervention across the organisation.
