About us:
The Transition Care Team provides a quality low intensity allied health intervention, nursing and case management service, to community-based clients post-discharge from acute and sub-acute hospital settings. Clients will have been recently assessed as eligible for residential care and appropriate for Transition

About your role:
In this position you will…..
- Provide consistent, safe and efficient occupational therapy and case management services, within a multidisciplinary team, with infrequent need for direct clinical practice supervision.
- Demonstrate resilience and problem-solving abilities while working in varied and demanding caseload.
- Provision of general clinical advice to professional and operational supervisors, relevant service managers and other stakeholders including non-government organisations and remote community agencies within Far North Queensland, regarding service delivery and case management on a regular basis and with infrequent need for supervision.
