About the Role:
The Complex Care Coordinator utilises specialised nursing competencies, coordinating the delivery of direct and indirect, advanced client-centred care, to high risk, complex client groups with chronic disease.
The aim of the Complex Care Coordinator is to improve the client journey and flow through Central Queensland Hospital and Health Service (CQHHS), minimising hospital presentations and ensuring the appropriate transition of ongoing client management in the community.
Salary Information:
The total remuneration value of up to $112,692 p.a., is comprised of a salary between $92,239 and $98,769 p.a., up to 12.75% employer superannuation contribution and 17.5% leave loading. (NRG6(1))

It is a condition of employment for this role for the employee to be, and remain, vaccinated against vaccine preventable diseases as listed on the Role Description.
