The Nursing Perioperative and Ambulatory Care at the Mackay Base Hospital is seeking applications for a Clinical Nurse Consultant (Post Operative Discharge Support) to join their team on a permanent basis.

Your opportunity
The purpose of this role is to demonstrate accountability at an advanced practice level for managing and coordinating ongoing care plans for relevant client group within geographical area and for the provision of nursing leadership specialist advice, consultation, and education to inter-professional team members as relevant to the specific context of the role.
About the Team
The Mackay HHS Post Operative Discharge Support Service (PODSS) is a rapid access model for post operative surgical care that provides alternate pathways to access specialist care for post operative Elective surgery patients experiencing complications to streamline the patient journey. The service covers all specialties and supports a continuity of care model to reduce representations. This service works collaboratively with multiple stakeholders across the surgical stream and provides ED avoidance strategies to assist with patient flow throughout the hospital providing an improved patient experience.
