Role details

The Home and Community Care (Mackay) at the Carlyle Community Health Centre is seeking applications for an Assistant Home Care Coordinator to join their team on a temporary basis.

Your opportunity

To assist in the coordination, rostering, implementation, monitoring and reviewing of a range of flexible home-based support services to eligible clients of the Commonwealth Home Support Program (CHSP) and Home Care Packages (HCP), with the aim of enhancing the independence of clients. This is to be in line with care plans developed for each client.

About the Team

Mackay District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible and goal orientated basic support services. Our service is also an Approved Aged Care Provider, providing Home Packages to eligible consumers under Client Directed Care.

About our Hospital and Health Service

Mackay Hospital and Health Service has 6 hospitals, 2 multi-purpose health services and 4 community health centres that employ more than 3,700 staff. The district covers an area of approximately 90,000km2 and provides services to patients and their families in Bowen, Cannonvale, Clermont, Collinsville, Dysart, Glenden, Mackay, Middlemount, Moranbah, Proserpine and Sarina.
