Project Street Cred is a collaborative initiative involving a range of government and non-government community agency including but not limited to Queensland Police, Gold Coast Youth Service, and Child Safety. The program engages with high-risk young people in community settings, currently operating on Thursday evenings. In this capacity, you will serve as the primary liaison between Child Safety and Project Street Cred, facilitate the exchange of information between partner agencies to ensure coordinated service delivery, maintain accurate and timely records of program activities and outcomes, conduct follow-up actions with young people associated with Child Safety, and build and maintain strong, collaborative relationships with both young people and partner agencies to support positive outcomes.

Your responsibilities

• Effectively and positively engage with children, families, carers and service providers (including both government and non-government agencies) as required to support children and parents to meet their identified case plan goals.
• Provide individualised, high quality, and culturally appropriate supports to children and families subject to tertiary child protection intervention including, but not limited to; facilitation of positive family connection, referrals and advocacy for services, parent coaching and support of transition from care plans.
• Work collaboratively with key stakeholders, children and parents to support the establishment and strengthening of effective safety and support networks, including connection to extended family and community.
• Provide high quality, comprehensive casework support and advice to assist in informing decisions about the safety and protective needs of children where complex child protection concerns are present.
• Participate effectively as a team member in a multidisciplinary team to deliver well-planned family support services to vulnerable children and families.
• Actively contribute to a culture of continuous learning, training and professional development, to ensure practice knowledge and skills are contemporary and evidence-based.
• Prepare and maintain quality case records in accordance with departmental case management requirements.
