About you:
In your new role you will:
- Exercise independent clinical judgement and more complex decision making in areas such as psychosocial assessments; counselling; crisis intervention and bereavement support in both inpatient and community settings for patients with a life limiting illness and including families and carers.
- Provide a leadership role in the clinical assessment and intervention planning for service recipients from a Social Work and multidisciplinary team perspective, ensuring timely identification of client's needs, service provision, and discharge planning within the work unit or professional team.
- Ensure patients and other staff are treated fairly and with mutual respect and understanding, regardless of cultural, religious, ethnic and linguistic backgrounds.
