You are invited to join a dynamic team with First Nations Cancer Care Survivorship, Aboriginal and Torres Strait Islander Health Services as Occupational Therapist/Physiotherapist – Senior (Health Practioner). In this role as members of the First Nations Cancer Care Wellness Service, develop, implement and deliver an innovative and integrated approach to care co-ordination for cancer survivorship across the care continuum including discipline specific clinical input and provide follow up with appropriate community liaison as directed by the Clinical Nurse Consultant.

About your role:
- Deliver quality, patient focused care co-ordination utilising specialist contemporary knowledge and recent clinical experience in the delivery of integrated multidisciplinary care and care co-ordination.
- Deliver high level discipline specific clinical and consultancy services across all streams of First Nations Cancer Care and Wellness Service. Recognised expertise, knowledge and skills are obtained through significant professional development activities, post-graduate education or formal qualification(s).
- Utilises evidence-based practice and professional judgement for a complex clinical caseload, to facilitate novel multidisciplinary clinical decisions and management with minimal clinical practice supervision.
