An exciting opportunity currently exists for you to join the Older Persons Subacute and Rehabilitation Services as a Social Worker - Senior (Palliative Care). You will deliver high level clinical Social Work services for clients of the Palliative Care Service to ensure optimal assessment, clinical intervention, discharge planning, end of life care and bereavement support as part of a multidisciplinary team providing specialised palliative care.

About your role:
- Apply high level professional judgement and decision making to manage a complex caseload, adapting routine and non-routine clinical practice based on valid and reliable clinical evidence with minimal direct clinical practice supervision.
- Develop clinical practice knowledge and expertise through active learning within health and the Social Work Service and by engaging in professional development activities and professional supervision in accordance with the Australian Association of Social Workers (AASW) code of ethics and AASW professional practice standards.
- Provide a leadership role in the clinical assessment and intervention planning for service recipients from a Social Work and multidisciplinary team perspective, ensuring timely identification of client's needs, service provision, and discharge planning within the work unit or professional team.
- Lead and manage staff by fostering and committing to patient safety and quality in the delivery of health care by maintaining and evaluating safety and quality practices and initiatives.
