About your opportunity:
In this position you will
- Contribute to an innovative evidence-based model of health care for aged care clients with complex health care needs.
- Effectively communicate at all levels across the organisation to promote effective team functioning.
- Deliver quality, patient focused care co-ordination utilising specialist contemporary knowledge and recent clinical experience for the management of the frail and elderly in a community environment with the primary focus of establishing sustainable supports for the prevention of functional decline and avoidance of hospital admission.
