The Commonwealth Home Support Programme (CHSP) is a multi-disciplinary team servicing the Cassowary Hub area which includes three hospitals, Babinda, Innisfail, and Tully. The vision of CHSP is to identify and assist the frail older person, who is at risk of early or premature admission into an acute or long-term health facility, to remain living safely and independently in their own home for longer.

About your opportunity:

In this position you will….. 

  • Effectively communicate with clients, carers, CHSP & CACH clinicians and staff, answering client enquiries via the telephone, directing enquiries to the appropriate staff within acceptable time frames.
  • Maintain a working knowledge of and use office systems including word processing, spreadsheet applications, data base management and record keeping.
  • Assist in maintaining accurate and appropriate record keeping systems for all relevant client and staff files.
