About the role
- The purpose of this role is to assist in the coordination of personal care services and ongoing referrals to Post Acute Care Service (PACS) eligible clients. Clients will be supported to enhance their functional independence through time limited intervention.
- The Post Acute Care Service operates seven days a week with extended opening hours – 0730 hours to 1800 hours seven days per week including public holidays. Further extension of operating hours may occur based on client and health service need.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Assist in the coordination and allocation of resources to provide services using a model of care that promotes each client's opportunity to maximise their functional capacity within Post Acute Care Service.
- Provide and assist clinicians to deliver services including attending referrals to other service providers and health professionals both within the department and with external stakeholders and clients. Support the management of the Personal Care Worker workforce including recruitment and selection, occupational health and safety and training of development.
- Work autonomously to schedule and roster appropriately skilled staff and relief staff that meet strict deadlines and timeframes and resolve day to day operational issues regarding procedures and work practices while maintaining high work standards.
- Responsible for data management and maintaining accurate records using a number of databases that support the service.
