About the role
Permanent ful time opportunity to deliver high quality outreach support to clients of the Diabetes and Chronic Disease Services, targeting Aboriginal and Torres Strait Islander people. This role encompasses planning, accessing, and delivering specialised health care programs, assessing, and treating clients, maintaining health care systems, and providing health education programs.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Demonstrated advanced-level clinical skills in conducting health assessments, community screenings, and primary health interventions, with a specific focus on managing and supporting individuals living with Diabetes.
- Lead high level communication and coordination efforts with endocrinologists, nurse practitioners, dieticians, exercise physiologists, social work and psychology, and other healthcare professionals to ensure that diabetes services for Aboriginal and Torres Strait Islander clients are culturally safe and appropriately tailored.
- Coordinate diabetes clinics, manage complex client care plans, and ensure that outcomes are accurately documented using patient information systems. This includes comprehensive tracking of chronic diabetes progression, treatment efficacy, and client responses to intervention.
