About the role
Support older people who have been discharged from hospital who require long term accommodation in a residential aged care facility and who are awaiting access to a facility. Interim Care supports the individual to make an informed choice about future accommodation needs. The area provides short term support and active management of older people at the interface of the acute / subacute and residential aged care sectors. Care provision in Interim Care is time limited and not expected to exceed a four-week period.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Provide leadership, communication and support effective teamwork among the CA3's to ensure service delivery is maintained at a high level, with all Clinical Task Instructions up to date and training provided to all CA.
- Identify potential changes needed in treatment programs using results from standardised assessment tools and gain endorsement to make these changes following discussion with the supervising Allied Health Professional.
- Support, mentor and supervise less experienced Clinical Assistants including teaching and assessment of generic competencies and delegated tasks.
