Transition Pathways Officer - Student Child and Family Connect

Transition Pathways Officers work with the Regional Youth Engagement Service to complement the role of state schools in providing safe, supportive and inclusive schools for many young Queenslanders, supporting young people to re-engage back into school or pursue a pathway to employment or training.

This role focusses on working in partnership with a range of government agencies, schools, registered training organisations, community-based organisations and other stakeholders, help young people who have become disengaged to re-engage in education or pursue a pathway to training or employment. The Transition Pathways Officer position is offered on a permanent full-time basis, and will be based in our Gladstone office.

About the Role

As the Gladstone Transition Pathways Officer, you will have responsibility for supporting students who have exited school prior to the completion of Year 12 to ensure they remain in an eligible option. The successful applicant will demonstrate the ability to develop professional relationships with young people and their families, education and training providers and other key stakeholders, as well as an understanding of case management processes.

Responsibilities include, but are not limited to:

  • Support schools to develop and refine departmental policies, practices and procedures relating to students exiting school prior to the completion of Year 12 to ensure they remain in an eligible option.
  • Provide support to schools that will assist the development of successful retention and transition strategies for ‘at risk' students.
  • Provide appropriate support and intervention to young people and their families to assist in the transition of young people into a viable education, training or employment option.
  • Facilitate the development of a school to post school transition plan for young people who leave school before the completion of Year 12 and provide appropriate case management and support as required.
