Intensive Education Case Manager - Central Queensland Region

As one of the Intensive Education Case Managers, you will work collaboratively within a multidisciplinary team to deliver intensive case management, provide transition support, and manage education, intervention, and prevention initiatives for students who are at risk of disengagement.
Your focus will be on students who are at risk of disengaging, have already disengaged, or are at risk of becoming involved in the youth justice system.
This role is based in Rockhampton and reports to the Manager of the Intensive Education Case Manager Initiative for the Central Queensland Region.

Who we are looking for
The ideal candidate will demonstrate a proven ability to establish and sustain effective partnerships with young people, parents/carers, school principals and other stakeholders in order to deliver tailored, intensive case management in alignment with legislative, policy, and procedural requirements to support students at risk of disengaging, or who have disengaged, from education.
About the role
As an Intensive Education Case Manager, you will play a pivotal role in the development and coordination of individualised case management plans in collaboration with key stakeholders. These plans may include behaviour support plans, risk assessments, education and transition plans and referrals to appropriate services.
Your role
Responsibilities include:
- Provide intensive case management that may require educational and behaviour intervention for students at risk of disengagement from education, in accordance with legislative, policy and procedural requirements.
- Establish and sustain effective partnerships with young people, parents/carers and the school principal during and beyond the case management process and initiate referrals and access to support.
- Analyse information and data to gain an understanding of the young person's strengths and provide strategic advice on identified concerns to inform effective case-management, integrated support planning and inform service enhancements.
