What's involved – The Nurse Navigator Service is a state-wide model of care designed to provide holistic support to patients with complex health conditions requiring an expert level of healthcare coordination. This integrated and proactive approach to health care service delivery, helps ensure that patients receive the most appropriate service when and where they need it.
The Nurse Navigator is an experienced nurse with expert clinical knowledge and in-depth understanding of the health system who employs advanced clinical and professional practice capabilities to deliver a positive patient-centred health experience for individuals. The Nurse Navigator works across system boundaries to be a patient advocate, help improve health literacy and quality of life, reduce service fragmentation, facilitate improved patient outcomes, and create partnerships with other health care professionals involved in a patient's care.
Your role will be based in Barcaldine with support and travel to other facilities and towns within the eastern hub including Alpha, Aramac, Jericho, Muttaburra. There are four (4) Nurse Navigator roles within the team and will be required to backfill across each role to facilitate continuity of care in Blackall, Longreach and Winton hubs.
As a member of the Community Health Team, the role will work collaboratively and more broadly with the team and across Central West HHS to ensure continued focus on strategic priorities and delivery of operational imperatives.
