What your day will look like….
You'll be at the centre of patient flow, coordinating discharge plans from the moment of admission, working collaboratively across teams to ensure safe transitions from hospital to home or community care. Drawing on your advanced clinical knowledge, you'll liaise with internal and external stakeholders, navigate aged care and NDIS pathways, and champion best-practice care outcomes. From mentoring colleagues to contributing to quality improvements and influencing change, your day will be as impactful as it is varied—guided by compassion, clinical excellence, and a commitment to holistic care.
