Your day will be filled with meaningful connection and purposeful action as you support aged care residents through high-level assessments, care planning, and clinical interventions. You'll collaborate closely with multidisciplinary teams, families, and external partners to deliver thoughtful, evidence-informed care that honours dignity and wellbeing. Whether developing care protocols, leading case management, or advocating for service improvement, your expertise will shape positive outcomes for those who need it most. Each interaction will reflect your compassion, leadership, and commitment to excellence.
