The Transition Support Service supports Aboriginal and Torres Strait Islander students in remote Aboriginal and Torres Strait Islander communities, where there is limited or no access to secondary education, to transition into distance education, boarding schools, or schools away from home while living in private hostels, to complete secondary education.

As the Transition Support Teacher, you will develop and maintain strong networks and inter-agency partnerships with a focus on providing differentiated supports and interventions for students and families from remote Aboriginal and Torres Strait Islander communities while transitioning into distance education, boarding schools, or private hostels.
Teachers play a key role in the delivery of quality education to students and will contribute towards:
- Productive partnerships effecting the service delivery of the Partnerships and Programs Division's goals
- Excellence in teaching and learning curriculum planning and implementation
- Future-focused pathways helping students to find purpose and fulfilment
- Responsive and progress-oriented strategic management
- Fair and equitable work practices and
- Responsible school management.
Applicants for these positions must possess:

- Current full registration or current provisional registration with eligibility for full registration, as a teacher in Queensland. Pre-service/graduate teachers must have been granted provisional registration prior to commencing employment as a teacher.
- In addition to registration, teachers working in Education and Care Services (including State Delivered Kindergarten programs) require an exemption card issued by Blue Card Services.
- Employment for pre-service/graduate teachers will be subject to the successful completion of a relevant teaching qualification and meeting the minimum requirements as a teacher with the department
