About Geriatric Evaluation and Management / Acute Cognitive Unit
The Geriatric Evaluation and Management Unit (GEM) provides comprehensive care for older patients, offering multidisciplinary support for both clinical issues functional rehabilitation which also includes an Acute Cognitive Unit (ACU) dedicated to managing patients living with dementia and experiencing severe Behavioural and psychological symptoms of dementia (BPSD), providing a highly structured environment with psychogeriatric support.
About your new role
As a valued member of the GEM and ACU team, you will deliver comprehensive, person-centred, and evidence-based care within a supportive, multidisciplinary environment. Your focus will be on promoting the wellbeing of older adults by partnering closely with patients and their families to reduce the impact of hospitalisation and improve quality of life. This includes
- Demonstrating expert knowledge in the care of the general medical patients, older persons, comorbidities, and the aging process
- Working collaboratively with the Service Line Nurse Educator to maintain a quality service
- Providing advanced clinical advice and coaching at the expert end of the scope of nursing continuum to support education of patients and family / carers promoting health literacy
- Managing patient flow within the unit coordinating admissions, transfers and discharges
Qualifications
- A Bachelor of Nursing or equivalent
- Registration with the Nursing Midwifery Board of Australia
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

Salary
- Permanent Full time and Permanent Part Time (multiple positions, hours negotiable)
- Temporary Full time and Temporary Part time (multiple positions, hours negotiable, up to 12 months with possibility of extension)
- Base salary ranging from $107 960 - $115 604 per annum or $54.44 - $58.30 per hour
