The Role
An opportunity has become available for a Social Worker - Senior (Older Persons Mental Health Community) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Provide high level clinical mental health services, in accordance with Social Work professional evidence-based assessments, intervention and discharge planning
- Utilise high level communication and develop effective professional relationships to work collaboratively with multi-disciplinary staff, consumers, families, carers, and internal/external stakeholders
- Develop and lead quality improvement activities to build capacity within the Older Persons Mental Health Team/Social Work for improved clinical practice, and continuity of care
- Participate in lifelong learning for Mental Health and Social Work professional development, engaging and sharing high level specialised Social Work knowledge and skill
About you
To be successful in this position you will require:
- A tertiary degree in Social Work
- Eligible for membership to the Australian Association of Social Workers (AASW)
- Driver licence
- Blue card / Working with children check (GCH will cover cost)
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

Salary
- Permanent part time (64 hrs p.f)
- Base salary ranging from $64.64 - $69.55 per hour
About Us
The Older Persons Mental Health Community Team service provides specialised, community-based multi-disciplinary services, including mental health assessment and treatment, for adults over the age of 65 years (>55 years for Aboriginal and Torres Strait Islander) who have a severe and complex mental illness, which is complicated by problems related to ageing.
The service engages with consumers in their own home or residential aged care facilities and works collaboratively with specialist aged care services. Support is provided to consumers, their families, and carers across the broad continuum of care, including facilitating smooth transition to other appropriate services.
