The Role
An opportunity has become available for a Clinical Nurse Consultant (Transition and Flow Coordinator) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
You will
- Coordinate mental health care transitions of patients across community teams
- Assess and plan care for complex, high-risk consumers
- Collaborate with Emergency Department (ED), inpatient, and community teams daily to achieve best practice outcomes
- Lead quality initiatives to improve care and service delivery
- Provide high level guidance, leadership and advice to community mental health services in relation to key performance indicators and data for consumer flow and experience
About you
To be successful in this position you will require:
- A Bachelor of Nursing or equivalent
- Registration with the Nursing Midwifery Board of Australia
- Credentialed through the College of Mental Health Nurses (or be prepared to work towards a credential in Mental Health Nursing)
- A Masters or higher qualification within a relevant area is highly desirable
- Driver's Licence
- Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
- Tuberculosis screening, to identify if the tuberculosis vaccination may be required
- Declaration of serious discipline history must be disclosed
- Criminal History Screening will be required for new employees (GCH will cover cost)

- Permanent full time
- Base salary ranging from $132 562 - $143 924 per annum

The Continuing Care Community Service Line is a tertiary service that operates across multiple locations in the Gold Coast Hospital and Health Service. Our service is available to adults experiencing a mental illness who are 18 plus years and their families who live in the Gold Coast region. Our services offer multi-disciplinary teams who are experienced in understanding and working with complex mental health issues experienced by adults and their families.
