The Role

An opportunity has become available for a Identified First Nations Nurse Navigator (Chronic Disease – Aboriginal and Torres Strait Islander) to join Gold Coast Health (GCH) where the focus of our 12 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.

You will

  • Work as the interface between GCHHS services, external culturally appropriate services and the Aboriginal and Torres Strait Islander Health Team
  • Provide high quality, evidence-based nursing care to improve the health and wellbeing of Aboriginal and Torres Strait Islander persons 
  • Identify and address fragmentation for complex clients who have multiple specialty touchpoints working towards better coordinated care for the individual
  • Assist clients in navigating the health system and transitioning between acute and community settings

About you

To be successful in this position you will require:

  • Bachelor of Nursing or equivalent  
  • Registration with the Nursing Midwifery Board of Australia
  • Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
  • Tuberculosis screening, to identify if the tuberculosis vaccination may be required
  • Declaration of serious discipline history must be disclosed
  • Criminal History Screening will be required for new employees (GCH will cover cost)

Salary

  • Permanent full time
  • Base salary ranging from $132 562 - $143 924 per annum

About Us

The Nurse Navigator position is an identified role within the Aboriginal and Torres Strait Islander arm of Chronic Disease & Post-Acute Programs (CD&PAP). The Nurse Navigator role is Gold Coast Hospital and Health Service (GCHHS) wide, based at Robina Health Precinct and is co-located with the Aboriginal and Torres Strait Islander Health Team.

Aboriginal and Torres Strait Islander programs include Better Cardiac Care, Mungulli Chronic Disease Service and Heart Health clinics. These community-based services are aimed at supporting self-management and rehabilitation for primary and secondary prevention. The service is designed to provide health equity to First Nations people and is supported by a multidisciplinary team whose focus is on chronic and complex conditions.
