About the Transition Care Program 

The Transition Care Program is a goal oriented restorative program that provides short term and active management for older people who have been hospitalised and who require more time and support in a non-hospital environment to complete their recovery. It is a flexible care program that offers case management along with low intensity rehabilitation, nursing, personal care, and other support services and may be delivered in a community or residential package of care. 

About Your New Role 

As a compassionate and skilled Physiotherapist, you will play a vital role in supporting older patients during their recovery journey at home or within community settings. This position involves conducting home visits, providing tailored support, and working in accordance with professional standards. You will collaborate with patients, families, and multidisciplinary teams to establish community supports, connect individuals with essential services, and identify suitable residential care options for seamless transitions upon program discharge. 

Your compassion and expertise will make a meaningful difference in the lives of those you serve.

Key Responsibilities

Provide professional level clinical and problem-solving skills with specific reference to care coordination
Provide case management services including the coordination of the client's assessment, care planning, and discharge planning
Contribute to multidisciplinary case conferences, and departmental team meetings
Undertake community and home visits to our elderly clients

Qualifications and Experience

Tertiary degree in Physiotherapy from a recognised tertiary institution
Aged Care Check (GCH will cover cost) 
Driver Licence
Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
Tuberculosis screening, to identify if the tuberculosis vaccination may be required
Declaration of serious discipline history must be disclosed

Salary 

  • Temporary full time or part time (several positions, up to 12 months with the possibility of extension, hours negotiable)
  • Base salary ranging from $79 051 - $116 189 per annum, $39.86 – 58.59 per hour
