As a Registered Nurse (Self-Management of Chronic Conditions) with the Health Contact Centre you will contribute to the education and empowerment of people to make informed choices about their health and health care, promoting better health through the delivery of evidence-based clinical practice. You will be responsible and accountable for delivering excellent patient focused, evidence-based nursing care in a specified field of practice (chronic disease management). What you'll do
The Health Contact Centre (HCC) is part of the Queensland Government's commitment to addressing the growing demand in health services in Queensland. The HCC operates 24 hours a day, seven days a week delivering confidential telephone assessment and information services to the people of Queensland. The HCC strives to educate and empower people to make informed choices about their health and health care, promoting better health through the delivery of evidence-based clinical practice. As a Registered Nurse (Self-Management of Chronic Conditions) you will work as part of a multidiscplinary team and be responsible and accountable for delivering excellent patient focused, evidence-based nursing care in a
specified field of practice (chronic disease management).
About you
We are searching for someone who has:
- Registration as a Registered Nurse under the Health Practitioner Regulation National Law Act 2009 with a current annual practicing certificate from the Nursing and Midwifery Board of Australia.
- Specialised nursing knowledge and skills in cardiac rehabilitation/secondary prevention, diabetes education, or respiratory rehabilitation.
- Effective computer skills and the ability to acquire significant skills and knowledge relevant to your role in a short timeframe.
- Ability to travel locally and work across nursing services within the HCC.
