About the role

Permanent full time opportunity to deliver high level, evidence based social work clinical services and leadership, as a member of a multidisciplinary team to consumers with complex mental illnesses in a specialised residential facility /rehabilitation unit, to facilitate rehabilitation, recovery, and transition to independent living.  Social Work services will also be provided to families, carers and significant others with a focus on planning and provision of psychosocial care and therapeutic interventions within recovery and brief intervention frameworks.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

• Apply high-level problem-solving skills to support the care co-ordination and discharge for consumers admitted to Secure Mental Health Rehabilitation Unit, and liaise with other healthcare services, government and community agencies including facilitation of stakeholder meetings.
• Engage in varied and complex inter-sectoral linking and liaison with relevant stakeholders and partnerships, including communication and interpretation of complex clinical information to secure the service needs of residents.
• Provide clinical practice supervision and education to Social Work students, HP3 Social Workers and clinical support to staff to ensure maintenance of professional clinical standards within the service.
