About the role
Permanent full time opportunity to deliver high-level clinical services and leadership as a member of an extended hours, multidisciplinary community mental health team to consumers with a moderate to severe level of mental illness, who are experiencing homelessness. Provide specialist mental health assessment and intervention, and manage a high-volume and complex caseload, focused on consumers transitioning between acute, community, other hospital health services and social support services to ensure continuity of care.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Undertake high level Social Work/ Psychology/Occupational Therapy and case management duties within the work unit with infrequent need for direct clinical practice supervision.
- Engage in varied and complex inter-sectoral linking and liaison with relevant stakeholders and partnerships, including communication and interpretation of complex clinical information to secure the service needs of consumers.
- Deliver clinical education and clinical practice supervision in accordance with the MHAODS supervision guidelines to less experienced Allied Health practitioners, students, and members of the multidisciplinary team.
