Social Work Senior (Pathways)

Allied Health are looking for Senior Social Worker to join their team. The purpose of this role is to deliver senior social work services within the Metro South Health (MSH) Pathways Team to complex long-stay patients within all facilities across MSH. This position will operationally report to the Team Leader of the Pathways Team and professionally report to the Director of Social Work at the assigned facility; however, location of the position may alter depending on clinical need across MSH.

About the Role
- Utilise high level social work knowledge and skills in the areas of disability, aged care, guardianship, psychosocial assessment, complex system navigation and the relevant legislative frameworks to inform clinical practice and provide clinical advice to the interprofessional team and other relevant stakeholders regarding long stay inpatient discharge planning.
- Assist in the identification of long stay risk and facilitate clinical escalations within a patient safety and clinical governance framework to ensure that timely and effective solutions are addressed under the guidance of senior staff.
- Assist in the identification, development, implementation and/or review of clinical practices, procedures and protocols with a focus on NDIS and long stay patient pathways using the principles of evidence based practice under the guidance of senior staff.
